Evaluation plan: develop a detailed system to measure and track outcomes on a monthly and annual basis
Service delivery system: create and/or enhance the structure of your program(s) to maximize impact
Outcomes: craft appropriate, measurable outcomes to ensure your organization is able to effectively track and report the impact of your program(s)
Logic model: create a graphic depiction of the relationship between the activities, outputs, outcomes and impact of your program(s) to depict the relationship between your program’s activities and its intended impact
